Service Animal Documentation

Please read the following notes as it pertains to traveling with a trained service dog and required documentation:

For travel booked more than 48 hours prior to departure, the Service Animal Air Transportation Form must be submitted a minimum of 48 hours prior to your scheduled departure.
For travel booked less than 48 hours prior to departure, customers must present the Service Animal Air Transportation Form at the departure gate on the date of travel.
Only dogs will be accepted as a trained service animal in the airplane cabin.
Please retain a copy of the Service Animal Air Transportation Form, as it may be requested by Frontier team members at different points throughout your travel.
