• Medical exceptions – If you cannot travel due to a severe or communicable illness.
  Required Documents: Medical note from a doctor. See qualification criteria here
• Lost or stolen travel documents – Required Documents: Embassy receipt or Police report.
• Jury duty or a subpoena requiring your presence – Required Documents: Jury duty notice or copy of subpoena.
• Death of a travel companion or immediate family member – Immediate family members include a spouse, domestic partner, children, parents, siblings, grandparents, grandchildren, aunts, uncles, nieces, and nephews. Eligibility applies whether the relationship is biological, adoptive, step, in-law or domestic partner, foster, or ward/legal guardian.
  Required Documents:
  • Death certificate, OR
  • Letter from a funeral director on funeral home letterhead (must include the relationship of the deceased to the ticket holder, if applicable).
• Military change of orders – If you or your eligible travel companions are affected by military orders or obligations.
  Required Documents:
  • Deployment orders, OR
  • Letter from a Commanding Officer or Personnel Officer stating the passenger’s name, scheduled itinerary dates/times, and confirming inability to travel due to military obligations.
• Legal name changes to your existing booking or Frontier Miles account name – Changes due to marriage, divorce, adoption, or other legal events.
  Required Documents: Copy of the updated driver’s license or passport and one of the following:
  • Marriage license
  • Divorce decree
  • Court order
• Date of birth corrections to your existing booking or Frontier Miles account to match government-issued ID. If there is a typo or a correction, and a passenger books a child under 15 years old online with an incorrect date of birth, the birthdate can be updated, but the PNR must be linked to the accompanying adult at the same time. To process date of birth corrections, there can be no previous name change present on the booking.
  Required Documents: Copy of the updated driver’s license or passport.
